What to Bring to Your Consultation:
✔ Any recent X-rays from a previous dentist, if available
✔ A list of current medications you are taking
✔ Your dental insurance information and completed forms, if applicable

Important Notes:

  • Patients under the age of 18 must be accompanied by a parent or guardian.

  • Please inform our office of any medical conditions (e.g., diabetes, high blood pressure, artificial heart valves or joints, rheumatic fever) or medications you are taking (e.g., heart medications, aspirin, anticoagulants) before your appointment.
